Registration Process of Birth/ Death Certificate :

Birth and Death Certificate is the most essential and mandatory certificates for all purposes. To obtain these certificates are very necessary for all citizens. The Civil Registration Department in Haryana is governed under Registration of Births and Deaths Act 1969 and Haryana Registration of Births and Deaths Rules 2002 made there-under. The process of Registration of every birth and death is mandatory under these provisions.

Following persons have been made responsible to report the events to the concerned Registrar within 21 days of its occurrence :
1. Head of family hold in case of domiciliary event.
2. Head or Incharge of medical Institution for event occurred in hospitals.
3. Senior Head or Incharge of institution in case of event occurred in Hostel, Jail, dharamshala or lodging house.
4. In case of any new born child or dead body found deserted in public place the investigating officer of local police station.

Registration hierarchy notified for the implementation and supervision of the civil registration system is as below.

Registrar (Births and Deaths) Urban Area :
Secretary, Municipalities

Registrar (Births and Deaths) Rural Area :
Incharge Medical Officer
Primary Health Centres.

Reporting Forms :
Birth/death or still birth shall be reported on the prescribed forms  such as : Form No 1, Form No 2, or Form No 3 respectively. These forms are available on the official website.

Important Instructions  :
1.  A Birth, Death or Still Birth must  be reported for registration, within 21 days of its occurrence. Free copy of the certificate can be obtained at the same time and day.
2.  Birth and Death registration has to be done at the place of occurrence.
3.  Birth can be registered without a name. Entry of name of the child can be done free of cost within 12 months from the date of registration. After 12 months, the name can be registered by paying a fee of Rs.5/-.
4.  If a name cannot be registered after 15 years from the date of registration of the birth. For the births and Deaths, that have been registered prior to 1.1.1995, without a name, the name can be entered on or before 31.12.2010 and for all the births registered on or after 1.1.1995, the time limit is 15 years, from the date of registration.
5.  Once the Name will be entered cannot be changed at any circumstances.
6.  Any birth or death of which information is given to the registrars, after the expiry of  21 days, but within 30 days of occurrence, shall be registered on payment of a late-fee of Rs.2/- (Rupees two only) and penalty of Rs. 5/-.
7.  Any birth or death of which information is given to the Registrar, after 30 days of occurrence, but within 01 year shall be registered only with the written permission of District Registrar and on payment of late-fee Rs.5/- (Rupees five only) and penalty of Rs. 10/-.
8.  Any birth or death which has not been reported within 1 year of its occurrence, shall be registered only on orders of the Sub Divisional Magistrate and on payment of a late-fee of Rs.10/- (Rupees ten only) and penalty of Rs. 25/-.

    Dear Sir/Madam,
    Name of my daughter, born in 1988, has not since been entered in her Birth Certificate and my name and my wife’s name is also not correct.
    I want to enter the name of my daughter in her birth certificate with correct name of my wife and me.
    It is requested to kindly help and guide in the matter.
