How to Apply f License Renewal :
Steps for Renewal of E-Registration Form :
1.For License Renewal Click on License Renewal Tab

2.Enter Your PCPNDT No and click on submit.

3.All the Other information will be same as new License Registration Except payment
4.Payment for Renewal is :
For Hospital with Multiple Facility the renewal Amount is Rs.17,500 and for hospital with single Facility Rs.12,500
If your payment type is Demand Draft, you will have to purchase the D.D in favour of “Appropriate Authority, D. H. & F. W. O. BANGALORE”
and then submit the same with your application no and the email acknowledgement received to the DHO office, with in Seven Days. If your payment type selected is RTGS/NEFT,please transfer the amount to the following account no

Branch : New Thippasandra
Kindly scan and send us the Voucher along with your application no to process your application.
5.Submit.Click on submit button, on successful submission you will get an application no which will be used for further application tracking.

How to Track Application Status :
Steps to Track Application Status :
1.For Tracking status ,Click on Track status Tab
2.Enter Your Application No and click on submit.
3.It will Display the status along with error if any, like issue in some data or some document got rejected.
Upon which you can upload or do the Appropriate action

How to Apply for Adding a New Machine :
Steps to Apply for Adding a New Machine :
1.For Applying for Adding New Machine Click on Machine Inclusion Tab
2.Enter Your PCPNDT No and click on submit.
3.Add the New Machine Details.
4.Also Two Documents Needs to be uploaded
4.1.Scan Machine Invoice Copy.
4.2.Request Letter
5.On Successful Submission you will receive mail and message

How to Apply for Changing Hospital Name :
Steps to Apply for Changing Hospital Name :
1.Applying for Changing the Hospital Name, Click on Hospital Name Change Tab
2.Enter Your PCPNDT No and click on submit.
3.Enter New Hospital Name.
4.Two Documents Needs to be uploaded
4.1.Affidavit Copy.
4.2.Request Letter
5.On Successful Submit you will receive mail and message
